Initial Evaluation or 1 Hour Follow-up (Approx 60 minutes): $275.00
Follow-up appointments (Approx 30 minutes): $165.00
Additional fees may apply for appointments lasting more than the allotted scheduled time.
**Charges for professional services are based on time spent and the nature of the service provided. Provider will discuss fees with you.
** A Good Faith Estimate (GFE) is an estimated fee for services provided to the patient from the provider rendering services. The goal of the GFE is to limit the possibility of unexpected medical expenses
Insurance:

Patients are responsible for payment for all services provided by Innovative Psychiatry. Co-pays, deductibles or self-pay amounts are due at the time of visit. Unfortunately, insurance companies do not always provide clear, simple instructions and fee schedules to determine amounts due. I will assist in confirming benefits prior to your first visit, but I cannot always reply upon the accuracy of the information provided by the insurance company.
Please note that the benefits quoted to us by the insurance company are not a guarantee of payment. Payment by the insurance company is subject to the terms and conditions provided within the contract between the patient and the insurance company.
Please bring all insurance cards and personal identification with you to your first visit.
Many insurance plans also require patients or family to request and obtain prior authorization for mental health services/treatment. Please review your plan benefits prior to calling to make an appointment.
(A superbill can be provided for those with out-of-network benefits for insurances not listed above, upon request.)
Other:
Innovative Psychiatry Center has an administrative fee of $10-$100.00 for various letters, records, etc. depending on complexity.
Fees for consultation with attorneys, teachers, and other agencies will be discussed before the service is provided. Any telephone consultation fees added to your account should be paid at your next visit.
Frequently, patients request letters or completion of other reports to better inform schools, employers, or insurance reviewers about their treatment. No records are ever released without your written permission. Some requests may incur additional professional fees. Letters to endorse pets as “emotional support animals” (ESA) will not be considered.
Payment can be made by cash, check, or major credit card and is expected at the time of service unless prior arrangements have been made.
No-Show/Missed Appointment Fee:
The full cost of the visit will be applied to the account for all no-show/late cancellation.
It is my goal at Innovative Psychiatry to provide you accessible, high quality and timely mental health care. In order for us to assure convenience and accessibility to all of our patients, it is important that patients arrive timely for all scheduled appointments or cancel the appointment 24 hours in advance. This policy allows us to make better use of our available appointment times for those patients in need of psychiatric care and therapy.
A “No-Show/missed appointment” is an occurrence where someone does not show up for an appointment and does not cancel the appointment in advance of the scheduled date and time. If you do not show up for your appointment or do not cancel the appointment 24 hours in advance, we will record the appointment as a “No-Show/missed appointment” and a fee will be assessed to your account/card on file. This fee will not be submitted to your insurance; it will be charged to the patient. You will not be able to reschedule with a provider until your fee is paid.
Refill Requests:
Refill requests will require a follow up appointment. Provider and patient will discuss frequency and refills schedule. No refills for controlled substances will be authorized outside of your scheduled appointment time.
